Alternatively, you can complete and submit a medical history questionnaire,^* (or download a copy and send it to us at Asthma & Allergy Research Group, Mailbox 2, FREEPOST SCO 3742, Ninewells Hospital, Dundee, DD1 9XX) (NO STAMP REQUIRED). Once we receive this, we will contact you to make an appointment for you to come up to the department for a screening visit.
